All attendees must read and agree to the EVENT GUIDELINES. Please print and carefully read these guidelines before making your decision on attending this event. All necessary information is included in this document. Read it carefully.

If you agree to the Guidelines and wish to attend you will need the following forms:

Form #1: Event Registration Form
Form #2: Medical Information Form
Form #3: Event Authorization Agreement

All forms and registration fees must be neatly filled out and mailed to the DYNA office by the stated deadline. Please remember to include the registration fee with your forms.

DYNA Registration Fee:
$25.00 per person attending fee (non-refundable).
